there are random chance mobs spread through out the game, there is no formula they can be any single mob in the game, if you are 80 or above (best done around 100 - 150) they will level you instantly to 255. also go fight in naxxramas, the titans in the frostwyrm lair are random spawn from level 150 to 255.
